Complete the game on Normal to unlock Hard mode. You will start every game (including the main quest) with the level you reached before. You can go up to level 90 in Hard mode.
Unlike the Normal mode, the monsters will be a much higher level, the bosses are harder, and you can get items "unique" (orange) with very powerful bonuses.
